Abstract:

Objective

To investigate the effects of endoscopic thyroid surgery through oral vestibular approach on clinical, postoperative aesthetic and quality of life in patients with thyroid cancer.

Methods

A total of 100 patients with thyroid cancer diagnosed pathologically from January 2020 to March 2021 were selected and divided into observation group and control group by random number table method, with 50 patients in each group. The observation group received endoscopic thyroid cancer surgery through oral vestibular approach, while the control group received traditional approach. SPSS 23.0 statistical software was used to analyze the data. Perioperative indicators, postoperative pain score 24 h(VAS),cosmetic effect,THYCA-QoL score and other measurement data were expressed as(

xˉ
±s),and independent sample t test was used. Postoperative complications and other statistical data were compared by χ2 test. P<0.05 was considered statistically significant.

Results

The operation time of the observation group was longer than that of the control group, but the hospital stay was shorter, and the intraoperative blood loss and VAS score were lower than those of the control group(P<0.05). There was no significant difference in the number of intraoperative lymph nodes dissection between the two groups(P>0.05). There was no significant difference in the total complication rate between the observation group(28%)and the control group(32%),P>0.05. The score of Vancouver scar scale in observation group was significantly lower than that in control group 1 and 3 months after surgery(P<0.05);The THYCA-QoL score of the observation group was significantly lower than that of the control group at 3 and 6 months after operation(P<0.05).

Conclusion

Endoscopic thyroid surgery through oral vestibular approach has the advantages of less intraoperative bleeding, short hospital stay, less postoperative pain in the operative area, and low incidence of complications, and has the cosmetic effect of no postoperative neck scar, significantly improving the postoperative quality of life of patients.
