目的

探讨胃癌组织中富含半胱氨酸肠蛋白1(CRIP1)的表达及临床意义。

方法

选取2020年1月至2021年12月收治的60例胃癌患者作为研究对象。通过免疫组织化学法(IHC)和实时荧光定量PCR(qRT-PCR)检测胃癌组织及相应的癌旁正常组织(距癌组织边缘>5 cm)中CRIP1蛋白及mRNA的表达情况,将患者分为高表达组(n=50例)和低表组(n=10例),应用SPSS 20.0软件处理数据,临床病理特征中计量资料采用(

x ¯
±s)表示,行独立样本t检验;计数资料采用[例(%)]表示,其中等级计数资料行秩和检验,其他计数资料行χ2检验;采用Kaplan-Meier法绘制生存曲线,Log-Rank检验CRIP1不同表达与胃癌患者预后的关系,并绘制ROC曲线,计算AUC和约登指数,确定最佳截断点。P<0.05为差异有统计学意义。

结果

胃癌组织中CRIP1蛋白的阳性表达率显著高于癌旁正常组织(83.3% vs. 31.7%,P<0.05),且CRIP1 mRNA在胃癌组织中表达显著上调(P<0.05)。CRIP1高表达与浸润深度、pTMN分期及淋巴结转移显著相关(P<0.05)。CRIP1高表达组的累积总生存率及累积无病生存率显著低于CRIP1低表达组(48.0% vs. 80.0%,42.0% vs. 70.0%),差异有统计学意义(Log-Rank χ2=4.008、3.906,P=0.045、0.048)。通过绘制CRIP1 mRNA相对表达水平对胃癌预后预测的ROC曲线,计算AUC值为0.865(95%CI:0.831-0.896),其中CRIP1 mRNA表达水平对胃癌预后预测的最佳截断值为1.94时,灵敏度为88.7%,特异度为76.5%,约登指数为0.652。

结论

CRIP1在胃癌组织中呈高表达,且CRIP1高表达与胃癌的浸润深度、pTMN分期、淋巴结转移密切相关,其可能参与了胃癌的发生、发展及转移,并对胃癌患者的预后具有一定的预测价值。

Objective

To investigate the expression and clinical significance of cysteine-rich intestinal protein-1 (CRIP1) in gastric cancer.

Methods

Sixty patients with gastric cancer admitted from January 2020 to December 2021 were selected as the study objects. Immunohistochemical method (IHC) and real-time quantitative fluorescent PCR (qRT-PCR) were used to detect the expression of CRIP1 protein and mRNA in gastric cancer tissue and corresponding adjacent normal tissue (>5 cm from the margin of cancer tissue). SPSS 20.0 software was used to process the data, and the measurement data in clinicopathological characteristics were expressed as (

±s). Independent sample t test; The count data were represented by [cases (%)], in which Rank Sum test was used for rank count data and χ2 test was used for other count data. Kaplan-Meier method was used to draw the survival curve, and Log-Rank was used to test the relationship between different expressions of CRIP1 and the prognosis of gastric cancer patients. ROC curve was drawn, AUC and Jorden index were calculated, and the optimal cut-off point was determined. P<0.05 was considered statistically significant.

Results

The positive expression rate of CRIP1 protein in gastric cancer tissues was significantly higher than that in adjacent normal tissues (83.3% vs.31.7%, P<0.05), and the expression of CRIP1 mRNA in gastric cancer tissues was significantly up-regulated (P<0.05). The high expression of CRIP1 was significantly correlated with invasion depth, pTMN stage and lymph node metastasis (P<0.05). The cumulative overall survival rate and disease-free survival rate in the high expression group of CRIP1 were significantly lower than those in the low expression group (48.0% vs. 80.0%, 42.0% vs. 70.0%), and the difference was statistically significant (Log-Rank χ2=4.008, 3.906, P=0.045, 0.048). By drawing the ROC curve of CRIP1 mRNA relative expression level to predict the prognosis of gastric cancer, the AUC value was calculated as 0.865 (95%CI: 0.831-0.896),When the best cut-off value of CRIP1 mRNA expression level was 1.94, the sensitivity was 88.7%, the specificity was 76.5%, and the Yoden index was 0.652.

Conclusion

CRIP1 is highly expressed in gastric cancer tissues, and the high expression of CRIP1 is closely related to the depth of invasion, pTMN stage and lymph node metastasis of gastric cancer, which may be involved in the occurrence, development and metastasis of gastric cancer, and has certain predictive value for the prognosis of patients with gastric cancer.
