目的

探讨肥胖症合并2型糖尿病患者腹腔镜胃袖状切除术(LSG)后早期糖脂代谢的变化及其关系。

方法

回顾性分析2017年5月至2020年9月肥胖症合并2型糖尿病和高脂血症并行LSG的患者23例临床资料,收集术前、术后1、3及6个月的体重及糖尿病、血脂相关指标,采用SPSS 23.0对数据进行统计学分析,符合正态分布的计量资料采用(

xˉ
±s)表示,行t检验;不符合正态分布的采用[中位数(P25,P75)]表示,采用Wilcoxon符号秩和检验;相关性分析采用Pearson相关分析。P<0.05表示具有统计学意义。

结果

23例肥胖症合并2型糖尿病患者术前体重、体质量指数(BMI)、糖化血红蛋白(HbA1c)、空腹血糖(GLU)、高密度脂蛋白胆固醇(HDL-C)和胰岛素抵抗指数(HOMA-IR)与术后1个月、3个月、6个月相比差异均有统计学意义(P<0.05)。体重、BMI明显减低、GLU和HbA1c降至正常,HDL-C升高,差异均有统计学意义(P<0.05)。术后3个月和6个月血脂指标总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)与术前相比差异无统计学意义(P>0.05)。

结论

LSG后早期患者体重、血糖、HDL等指标得到了显著地改善。

Objective

To investigate the effect of early glycolipid metabolism in obese patients with type 2 diabetes after LSG surgery.

Methods

A retrospective analysis was performed on 23 patients who were obese and complicated with type 2 diabetes and underwent LSG surgery from May 2017 to September 2020. The relevant indexes of weight,diabetes and blood lipids before surgery,1,3 and 6 months after surgery were collected. SPSS 23.0 was used for statistical analysis of the data,and the measurement data were presented as(

±s)with normal distribution. Those who did not conform to normal distribution were represented by[median(P25,P75)]. Those who did not conform to normal distribution before and after surgery were analyzed by paired T-test,and those who did not were analyzed by Wilcoxon sign rank sum test. Pearson correlation analysis was used for correlation analysis,and P<0.05 was considered statistically significant.

Results

The 23 obese patients with type 2 diabetes mellitus had differences in preoperative weight,glycated hemoglobin(HbA1c),fasting blood glucose(GLU),high density lipoprotein(HDL),and insulin resistance index(HOMA-IR)compared with the above indicators 1,3 and 6 months after surgery There was statistical significance(P<0.05). Body weight and BMI were significantly reduced,GLU and HbA1c were normal,and HDL was increased,with statistical significance(P<0.05). There was no significant difference in serum lipid indexes CHDL,total cholesterol(CHDL),TG,and low density lipoprotein and other indicators(LDL)3 and 6 months after surgery(P>0.05).

Conclusions

The indexes of body weight,blood glucose and HDL were significantly improved in the early postoperative period of patients with LSG.
