目的

观察新辅助化疗联合手术治疗进展期胃癌的临床疗效。

方法

按照双盲随机数字表法将68例进展期胃癌患者分为新辅助化疗组和对照组各34例，两组患者均行腹腔镜手术治疗，新辅助化疗组患者术前行新辅助化疗，运用SPSS 19.0统计学软件进行分析，近期疗效比较、手术切除率以及根治性切除率、术后并发症、术后1年生存率比较采用卡方检验，P<0.05表示差异有统计学意义。

结果

新辅助化疗组总有效率为79.4%(27/34)，降期率为41.2%；对照组总有效率为47.1%(16/34)，降期率为14.7%。差异有统计学意义(P<0.05)；新辅助化疗组手术切除率和根治性切除率均明显高于对照组(P<0.05)；两组患者术后并发症发病率比较，差异无统计学意义(P>0.05)；新辅助化疗组患者术后1年生存率明显高于对照组，差异有统计学意义(P<0.05)。

结论

新辅助化疗联合手术治疗进展期胃癌可以有效提高手术效果和近期生存率，且毒副作用较轻，而远期效果还需进一步研究。

Objective

To explore the clinical effects for the patients with advanced gastric cancer by neoadjuvant chemotherapy combine with operation.

Methods

68 cases of advanced gastric carcinoma patients were randomly divided into neoadjuvant chemotherapy group(34 cases) and control group(34 cases). They were treated by laparoscopic surgery. The neoadjuvant chemotherapy group were treated by neoadjuvant chemotherapy before operation. The data were analyzed by SPSS 19.0, the clinical effects, the rate of excison, the rate of curative recetion, complication, survival rate were compared, P<0.05 showed than the difference was statistically significan.

Results

The total effective rate for the neoadjuvant chemotherapy group was 79.4%(27/34), drop period rate was 41.2%; The total effective rate for the control group was 47.1%(16/34), drop period rate was 14.7% (P<0.05); The surgical removal rate and radical removal rate for the neoadjuvant chemotherapy group were higher than the control group (P<0.05); There had no significant difference on the rate of complication (P>0.05); The survival rate 1 year for the neoadjuvant chemotherapy group were higher than the control group (P<0.05).

Conclusion

It can improve the operation result and recent survival rate for advanced gastric carcinoma patients with neoadjuvant chemotherapy and operation. It has less side effects, the long-term effects needs more research.
