“The Method of Tunnel” Laparoscopic anatomic resection of the left half of liver. This surgical approach is based on our new findings and profound understanding of liver anatomy in laparoscopic left hemihepatectomy. Therefore，this method is a new surgical method which breaks the principle of traditional laparoscopic left liver resection. The point is as follows：separate intraperitoneal adhesions and routinely place the he block band at the first hilum，dissociate the ligaments around the liver to appear the root of left hepatic vein. A tunnel over the liver was established between the middle hepatic vein and left hepatic vein after opening the liver parenchyma from the right side of the static root of the left liver to dissociate the right margin of the left liver to the side of foot. The left pedicle was dissected at the first hilum，close to the right side of the left hepatic pedicle and establish a tunnel beneath the liver toward the head to run through the tunnel under the field of vision. Through the tunnel，cut off the left pedicle and the left hepatic vein by the endoscope cutting closure to show the whole of middle hepatic vein from the posterior side. Finally the liver was quickly severed along the plane of left and right hepatic ischemia line and middle hepatic vein，and the blood vessels encountered in the liver section were ligated with synthetic clip and titanium clip.
