Modified Glisson pedicle transection “The Method of Tunnel” transumbilical single-port laparoscopic left hemihepatectomy

Abstract:

Our team has carried out transumbilical single-port laparoscopic left hemihepatectomy since 2020,and has completed 12 cases of transumbilical single-port laparoscopic left hemihepatectomy. In the process of exploring the single-port left liver resection,it was found that the modified Glisson pedicle transection "The Method of Tunnel" can significantly shorten the operation time and reduce the bleeding:firstly,establish a tunnel under the first hepatic portal,the right wall of the left hepatic pedicle and the left wall of the right hepatic pedicle are dissected. After confirming the safety of the right hepatic pedicle,using the cutting closure clamps and breaks the left hepatic pedicle,which can further reduce the bleeding and shorten the operation time. The modified Glisson pedicle transection "The Method of Tunnel" transumbilical single-port laparoscopic left hemihepatectomy significantly reduces the operation steps,shortens the operation time and reduces the bleeding compared with our previous single-port laparoscopic left liver resection via the anterior approach. Therefore,it is an operation worth exploring.
