Abstract:
Objective The results of hepatectomy based on Takasaki segmentation and Couinaud segmentation in the treatment of liver cancer.
Methods 86 patients with liver cancer admitted from January 2018 to December 2020 were selected and divided into Couinaud group and Takasaki group by random number table method, with 43 patients in each group. Anatomical hepatectomy was performed in both groups. Couinaud group was treated with Couinaud segmentation method, and the Takasaki group was treated with Takasaki segmentation method. The data were SPSS 22.0 was used for data analysis. Perioperative related indicators were expressed by(
±
s),and independent
t test was performed;Postoperative complications were counted by
χ2 test. Repeated measurement
ANOVA was performed for liver function index and other repeated measurement data.
Kaplan-Meier method and
Log-Rank test were used for survival analysis.
P<0.05 was considered statistically significant.
Results The operation time, intraoperative blood loss, intraoperative blood transfusion and postoperative hospital stay in Takasaki group were significantly lower than those in Couinaud group(P<0.05). There was no significant difference in hepatectomy volume between the two groups(P>0.05). The main effect of time was significant at total bilirubin(TBIL),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)levels(P<0.05),while the main effect of method was significant at TBIL,AST and ALT levels(P<0.05). There was no interaction between time and method on TBIL,AST and ALT levels(P>0.05). There was no significant difference in total postoperative complications and survival between the two groups(P>0.05).
Conclusion Compared with the Couimud subsection method,Takasaki subsection method can not only effectively shorten the operation time, reduce the intraoperative injury, improve the safety of the operation, but also benefit patients' rapid recovery and liver function recovery, and have a good prognosis.
