（1）Yellow-white borderline formed by the adhesion of the root of sigmoid mesocolon and the posterior peritoneum，from which the Toldt's space was entered，is exposed to the root of inferior mesenteric artery to complete D3 lymph node dissection；（2）Dissection between the rectal fascia propria and the anterior fascia of the hypogastric plexus can achieve the preservation of the pelvic autonomic nervous system intact and completion of the Total mesorectal excision.（3）An end-to-end anastomosis of sigmoid colon and rectum was performed totally laparoscopically.（4）Prophylactic loop ileostomy using continuous suture of dermis and seromuscular layer was performed in totally laparoscopic radical resection for rectal carcinoma.
