目的

探讨直肠黏膜渗液FH检测在非手术直肠癌患者局部疗效诊断的临床意义。

方法

选取2016年12月至2017年12月36例非手术直肠癌患者,在治疗前、治疗中、治疗后分别进行直肠黏膜渗液FH检测和肠镜检查,以肠镜病理结果为标准,采用SPSS17.0软件进行分析,肠镜和FH两种检测方法对比结果采用卡方检验;Kappa一致性系数检验考察两种检测方法检验结果的相关性和一致性,P<0.05差异具有统计学意义。

结果

在治疗前、治疗中、治疗后行直肠黏膜渗液FH检测及肠镜检查的检出率分别为97.2%、100%;55.6%、66.7%;27.8%、33.3%,P均>0.05,差异无统计学意义;在治疗前、治疗中、治疗后FH检测的阳性预测值为91.8%、93.0%和94.7%;治疗中、治疗后两种检测方法的相关性检测Kappa一致性系数分别为0.769和0.870,且P均<0.0001。

结论

直肠黏膜渗液FH检测在评估非手术直肠癌局部疗效中的作用和肠镜检查效果基本一致,有望为非手术直肠癌疗效评价提供更加简便、快捷的指标。

Objective

The purpose of this study was to investigate the clinical significance of FH detection of rectal mucosal exudates in the evaluation of local curative efficacy of non-operative rectal cancer.

Methods

A total of 36 patients with non-surgical rectal cancer from December 2016 to December 2017 were selected for rectal mucosa percolation FH test and colonoscopy before, during and after treatment. The results of colonoscopy were analyzed using SPSS17.0 software, and the results of colonoscopy and FH were compared using chi-square test. The Kappa consistency coefficient test was used to examine the correlation and consistency of the test results of the two detection methods, and P<0.05 was statistically significant.

Results

The detection rate of rectal mucosa percolation FH test and colonoscopy before, during and after treatment was 97.2%; 100%; 55.6% and 66.7%; 27.8%; 33.3%; all P>0.05, the difference was not statistically significant. The positive predictive values of FH before, during and after treatment were 91.8%, 93.0% and 94.7%. The Kappa consistency coefficient was 0.769 and 0.870 respectively, and all P<0.0001.

Conclusions

FH detection of rectal mucosal exudate and colonoscopy in the evaluation of local curative effect of non-operative rectal cancer is equivalent, which is expected to provide a more simple and rapid index for the evaluation of curative effect of non-operative rectal cancer.

摘要
