目的

对比全机器人手术与机器人辅助手术根治远端胃癌的近期疗效,评估全机器人远端胃大部切除术的安全性及可行性。

方法

回顾性分析2014年12月至2016年12月收治的行远端胃大部切除术的160例胃癌患者的临床资料,其中60例行全机器人远端胃大部切除术(TRDG组),100例行机器人辅助远端胃大部切除术(RADG组)。采用SPSS18.0软件进行统计分析,手术相关指标计量资料用(±s)表示,采用独立样本t检验,术后并发症采用χ2检验。P<0.05为差异有统计学意义。

结果

与RADG组相比,TRDG组手术时间、淋巴结清扫数目和术后并发症差异无统计学意义(P>0.05),但术中出血量,切口长度,术后通气时间以及住院时间均明显减小(P<0.05)。

结论

全机器人远端胃大部切除术安全可行,且与机器人辅助组相比,其对患者损伤更小,术后恢复更快,近期疗效显著。

Objective

To compare the short-term clinical outcome between total robotic distal gastrectomy (TRDG grop) and robot-assisted distal gastrectomy (RADG group) for distal gastric cancer, and to evaluate the safety and feasibility of total robotic surgery system.

Methods

From December 2014 to December 2016, clinical data of 60 cases in TRDG group and 100 cases in RADG group were analyzed retrospectively. Intraoperative indicators and postoperative outcome were compared between two groups.

Results

There were no significant differences in terms of anastomosis time and harvested lymph nodes between TRDG grop and RADG grop. However there were significant differences in terms of blood loss, incision length, flatus time and hospital stay between TRDG grop and RADG grop (P<0.05).

Conclusion

Compared with robot-assisted distal gastrectomy, total robotic distal gastrectomy surgery has advantages including less trauma, quicker postoperative recovery and satisfactory short-term outcome. Thus, it is feasible and safe for surgical treatment of distal gastric cancer.
