Comparison of the short-term efficacy and the safety of three kinds of surgical procedures for patients with cT1N0 thyroid cancer

Abstract:

Objective

To compare the short-term efficacy and the safety of three kinds of surgical procedures for patients with cT1N0 stage thyroid cancer.

Methods

A retrospective cohort study was conducted in 73 patients with cT1N0 thyroid cancer from January 2017 to May 2019, who were divided into three groups according to different surgical methods, including 25 patients in the open group, 24 patients in the whole areola group and 24 patients in the transoral group. Statistical analysis were performed by using SPSS 23.0 software. Measurement data, such as perioperative related indicators, visual analogue scale (VAS) and Vancouver Scar Scale (VSS) scores were expressed as (±s), and were examined by using One-way analysis of variance. Short-term efficacy and postoperative complications were analyzed by using χ2 test A P value <0.05 was considered as statistically significant difference.

Results

The total effective rate of 91.7% amd 87.5% in the trans-oral group and total areola group was higher than 64.0% in the open group (P<0.05); while the operation time in the trans-oral group and total areola group was longer than that in the open group (P<0.05) However, in terms of the intraoperative blood loss, postoperative drainage, and postoperative hospital stay were much lower than those in the open group respectively (P<0.05); the VAS score and VSS score at POD3 and 1 month in the oral group were less than those in the total areola group and the open group (P<0.05); The incidence of postoperative complications of 8.33% and 8.33% was much lower in the transoral group and the whole areola group than 32.0% in the open group (P<0.05).

Conclusion

Compared with open surgery, endoscopic thyroid surgery has better curative effect, fewer complications, and shorter hospital stay in the surgical treatment of cT1N0 stage thyroid cancer, and could be used as the preferred therapuetic treatment.
