Surgical management for 57 cases of intra-abdominal giant mesenchymal tumors

Abstract:

Objective

To summarize the clinical characteristics,surgical management and prognosis of intra-abdominal giant mesenchymal tumors.

Methods

Retrospective collection of qualified cases between Jan 2018 and Jun 2021 was performed. Demographics,clinical,surgical and pathological characteristics as well as prognosis were analyzed. Continuous variables with normal distribution were presented as mean ± sd,and Continuous variables with abnormal distribution were presented as median ± IQR.

Results

57 cases with abdominal giant mesenchymal tumors ≥10 cm in diameter and ≥1 kg in size were included,including 31 gastrointestinal stromal tumors,13 liposarcoma,2 leiomyosarcoma,2 rhabdomyosarcoma,2 lymphangioma,2 Schwannoma and the other 5 cases were collected. The on-set age was(55.8±13.9)years old,the diameter of tumor was(13.0±7.0)cm,the overall duration of surgery was(175.0±90.0)min,the intraoperative bleeding was(200.0±450.0)ml,intraoperative blood transfusion rate was 26.3%,the multifocal incidence was 22.8%,the en-bloc resection rate was 80.7%,the tumor invasion rate was 22.8%,the combined organ resection rate was 50.9%,the postoperative hospitalization was(11.0±5.0)d. During the(23.0±16.0)m follow-up period,4 cases(7.0%)deceased from tumor recurrence and metastasis.

Conclusion

Gastrointestinal stromal tumors and liposarcoma were the most common intra-abdominal giant mesenchymal tumors. Different types of tumors have different clinical characteristics and surgical treatment. Improving the understanding of these diseases is helpful to improve the clinical prognosis of these patients.
