Laparoscopic pancreaticoduodenectomy

Abstract:

Laparoscopic pancreaticoduodenectomy(LP),as the most difficult procedure in minimally invasive pancreatic surgery,had been performed in 2007 in our center. In recent years,the technical improvement of LP has promoted the development of minimally invasive pancreatic surgery,especially for robotic assisted laparoscopic pancreaticoduodenectomy. The main reason for the difficulty of LP in early stage,except for lack of a good team cooperation,was too much technical difficulties,including dividing duodenum from uncinate process. The mobilization of Treitz ligament was the break point of the operation,and the transection of the ulcinate process is the key to the operation. Therefore,fully exposure of the surgical field based on anatomy,good surgical cooperation,effective control of critical structure,familiarity and effective use of energy based instruments such as ultrasound forceps and Biclamp,and treatment of common postoperative bleeding,are prerequisites for LP. In this video,we showed the method of uncinate first dissection and removal of specimen in our center. Besides,the 301 pancreatojejunostomy and single layer biliary jejunostomy simplifies the anastomosis,which reduced the difficulty of gastrointestinal reconstruction in LP without decreasing surgical safety.
