Application of contrast-guided guidewire positioning combined with nano-carbon staining in the localization and characterization of breast cancer sentinel lymph nodes

Abstract:

Objective

To analysis the value of guided wire localization combined with Nano-carbon staining in SLN localization of breast cancer under the guidance of contrast-enhanced ultrasound.

Methods

Ninety patients with breast cancer who underwent surgery in our hospital from January 2017 to December 2018 were selected as subjects. Routine ultrasonography was performed in all patients after admission. During the examination, sulfur hexafluoride solution was injected into the subcutaneous tissue around the tumor at 3, 6, 9, 12 points or around the tumor. The specific location of sentinel lymph nodes was determined by lymphangiography and guidewire. The sentinel lymph nodes were stained by injecting nano-carbon suspension into the subcutaneous tissues of the tumors before operation. The sentinel lymph nodes were resected routinely during the operation, and the related data were counted according to the classification of pathological results. The data was analyzed by the SPSS 20.0 software. The number of lymph nodes, the rate of SLN metastasis and the rate of guided wire localization were measured by n (%), P <0.05 was considered to be statistically significant.

Results

Successful operation, complete resection of the lesion and the guided wire were completed in all 90 cases. Lymph node metastasis was found in 42 cases, and a total of 150 SLN were detected, 97 of which had metastasis of SLN. The black staining rate of metastatic lymph nodes was 88.66%(86/97), and the black staining rate of non-metastatic lymph nodes was 91.87%(226/246). There was no significant difference in the black staining rates (P>0.05). The rate of guided wire localization in patients with lymph node metastasis was 71.13%(69/97), which was significantly higher than that in patients without lymph node metastasis(51.03%(128/246)), (χ2=5.218, P<0.05). The sensitivity, specificity, positive predictive value and negative predictive value for SLN localization of breast cancer were 100.00%, 97.47%, 98.34% and 100%, respectively. The diagnostic efficiency was the highest.

Conclusion

Guided by contrast-enhanced ultrasound, guiding wire localization combined with nano-carbon staining has great value in the localization of breast cancer SLN. It has great significance for patients to make reasonable surgical plan and prognosis, and it is worth popularizing and applying in clinic.
