目的

探讨"三镜"联合在胆囊结石伴肝外胆管结石中的应用价值。

方法

回顾性分析2016年4月至2018年4月收治的胆囊结石伴肝外胆管结石患者122例，按照治疗方式不同将其分为两组，开腹组采用常规开腹胆囊切除术、胆总管探查取石术、T形管引流术联合；三镜组采用腹腔镜、十二指肠镜及胆道镜联合治疗。数据应用SPSS 18.0进行分析，体重、病史及治疗时间等用(±s)表示，采用独立t检验；疗效对比及并发症发生率以例(%)表示，采用χ²检验，P<0.05为差异有统计学意义。

结果

开腹组有效率为88.5%，三镜组有效率为96.8%，三镜组疗效高于开腹组(P<0.05)；开腹组并发症发生率为19.7%，明显高于三镜组发生率为6.4%，(P<0.05)。

结论

"三镜"联合在胆囊结石伴肝外胆管结石疗有效率高、并发症发生率低，值得在临床上推广。

Objective

To investigate the value of combination of three endoscopies in gallstones and extrahepatic bile duct stones.

Methods

A retrospective analysis of 122 patients with gallstones and extrahepatic bile duct stones admitted to the two hospitals from April 2016 to April 2018 was preformed. The paitients were divided into two groups according to different treatment methods. The open group was treated with conventional open cholecystectomy and common bile duct exploration combined with stone extraction and T-tube drainage; the three endoscopies group was treated with laparoscopy, duodenoscopy and choledochoscopy. Data were analyzed by SPSS 18.0. Body weight, medical history and treatment time were expressed by (±s), and compared with independent t test. The efficacy comparison and complication rate were expressed by example (%) and compared with χ² test, P<0.05, indicated the difference was statistically significant.

Results

The effective rate was 88.5% in the open group and 96.8% in the three endoscopies group. The efficacy of the three endoscopies group was higher than that in the open group (P<0.05). The complication rate in the open group was 19.7%, which was significantly higher than 6.4% of the three endoscopies group (P<0.05).

Conclusion

Combination of three endoscopies usedin gallbladder stones with extrahepatic bile duct stones has high efficiency and low complication rate, which is worthy of clinical promotion.
