目的

研究Geminin和细胞周期依赖性激酶4(CDK4)的表达与大肠癌生物学行为及预后的关系。

方法

收集2011年5月至2014年5月手术切除的大肠癌标本50例,应用免疫组织化学(SP法)检测Geminin和CDK4在50例大肠癌、20例大肠腺瘤及20例正常大肠黏膜组织中的表达。采用SPSS 13.0软件进行统计分析。采用χ2检验及Spearman相关性分析方法分析数据。P<0.05为差异有统计学意义。

结果

Geminin和CDK4在大肠癌组织、大肠腺瘤、正常大肠黏膜组织中的阳性表达率逐渐降低,各组间比较差异有统计学意义(P<0.05) 。Geminin和CDK4表达与大肠癌的分化程度、淋巴结转移、Dukes分期及浸润肌层的深度密切相关(P<0.05),且两者表达成明显正相关( P<0.05) 。术后3年生存率:Geminin表达阳性者23.8%(10/42)明显低于阴性者达62.5%(5/8);CDK4表达阳性者22.5%(9/40)明显低于阴性者为60.0%(6/10),差异均有显著统计学意义(P<0.05)。

结论

Geminin和CDK4的表达升高参与了大肠癌的发生、发展,对两者的检测可能作为临床判断大肠癌的恶性程度、转移及预后的重要参考指标。

Objective

To study the relationship between the expression of Geminin and cyclin dependent kinase 4 (CDK4) and the biological behavior and prognosis of colorectal carcinoma.

Methods

50 cases specimen of colorectal carcinoma and complete follow-up data were collected from May 2011 to May 2014. SP immunochemical method was performed to detect the expression of Geminin and CDK4 in 50 cases of colorectal carcinoma, 20 cases of colorectal adenomas and 20 cases of normal colorectal tissues. SPSS 13.0 software was used for statistical analysis. Chi square test and Spearman correlation analysis were used to analyze the data. P<0.05 was statistically significant for the difference.

Results

The positive rates of Geminin and CDK4 in colorectal carcinoma tissues, colorectal adenomas and normal colorectal mucosa tissues group were gradually decreased, and there were significant difference between each group (P<0.05). The expressions of Geminin and CDK4 were closely related to the degree of differentiation, lymph node metastasis, Duke’s stages and the deepness of invasion (P<0.05). The expression of Geminin was positively correlated with CDK4 (P<0.05). The 3 years survival rates after operation in positive expression groups of Geminin (23.8%) and CDK4 (22.5%) were significantly lower than those in negative expression groups (60.0%).

Conclusion

The abnormal expressions of Geminin and CDK4 may participate in the occurrence and development of colorectal carcinorma. The detection of both may be used as an important reference index for clinical judgement of malignancy, metastasis and prognosis of colorectal carcinoma.
